The Royal Kennel Club is holding a number of health screening clinics on 28 June, 6 September and 15 November from 09.30am - 4pm at The Royal Kennel Club Building, Stoneleigh Park, Stoneleigh, Kenilworth CV8 2LZ.

Please complete this form to request an appointment. Appointment spaces are limited and will be given on a first come, first served basis, with a booking secured once payment is made. We will then contact you with details of available appointment times and how to reserve one,  Please use one form per dog.
